Alternatively, if you order Online Survey you can use Mind Garden's Transform system - you input the names and email addresses of your participants or send out a link and the data is collected and scored automatically (raw scale scores by respondent). You can purchase a License to Reproduce/Administer in two ways:

After checkout you will receive an email containing instructions on how to download your PDF file. This file will include one copy of the instrument and scoring key, and a license to reproduce the purchased number.

A License to Reproduce will allow you to either conduct a paper and pencil survey, or re-type and reformat for use online.

You should order Mind Garden's Online Survey if...

... You want to use Mind Garden's Transform system to administer your survey online. No need to re-type and reformat. Transform will collect the data and can download it to you with raw scale scores by respondent.

With Transform, we do most of the work for you. You input the names and email addresses of your participants or send out a link. You monitor responses, send reminders and download your data anytime (.csv file). We can customize your survey by adding questions and even other survey instruments (on a fee basis).

STEP ONE: Confirm that you understand the limitations of our translations:

We offer no warranty or assurance of quality for these translations. The translations listed are of varying quality; many are done by individual researchers. While most of the translations are complete, some are only partial. Some contain versions from multiple translators. Very few have validation data. Most buyers are satisfied with what they receive and are pleased we do not charge extra for them.

I acknowledge that I will receive the translation free with the purchase of the English version. I understand the limitations outlined in the above paragraph.
